Welcome to the online portion of registration for St. John's Episcopal School, the Parents Association and Crusader Club. To begin the process, you will need the Family Login ID and Password previously emailed to you. If you can't find your login and password, please email cpatterson@stjohnsschool.org.

Remember, this online site is just one part in a two-step process. You must complete BOTH to ensure a complete registration.

STEP 1

SUBMIT MEDICAL INFORMATION – Print out, complete and return to school the following health forms; include copies of your child’s immunization records if your child is a new student or if your child has received immunizations since you last submitted them. Students with incomplete immunization, health and emergency contact records will not be permitted to begin school on Tuesday, August 21.

Immunization Records (REQUIRED AS STATED ABOVE)
Health History Form (REQUIRED FOR ALL STUDENTS)
Administration of Over-the-Counter Medication (REQUIRED FOR ALL STUDENTS)
Administration of Prescribed Medication (BASED ON INDIVIDUAL STUDENT NEED)

If your child has a severe allergy, asthma, diabetes or other signficant condition, the school requires that a physician's action care plan be on file in the clinic. To discuss this or any other concerns, please contact the clinic at clinic@stjohnsschool.org or 214-328-9131 ext. 109.

STEP 2

REGISTER ONLINE – Finish the process here by providing the requested information. The online process should take approximately 10 minutes. Your information will be saved if you need to quit and come back later, or if more than one parent wants to log in to select volunteer opportunities or purchase items from the Spirit Shop. Remember to check the Useful Links (upper right portion of page) for information such as important school dates, uniforms, a special letter to pre-k parents, health forms, and sixth through eighth grade textbooks.

Before you begin, please have the following on hand:

Phone numbers and addresses of pediatricians (REQUIRED FOR ALL STUDENTS)
Phone numbers for emergency contact friends and family (REQUIRED FOR ALL STUDENTS)
Your child's baptism date and church (required for Communion permission)
Family calendar (helpful when selecting volunteer opportunities)
Credit card (for PA Wishing Well and spirit items)
